Please read all the terms and conditions of this agreement with Canada Fetal Alcohol Spectrum Disorder Research Network (“CanFASD”) carefully. By clicking “I Accept”, or by using the CanFASD website, you agree to be bound by the terms and conditions of this agreement, even if you have not read them. It is important to read this entire agreement. In particular, it contains provisions that may limit your rights, such as the section entitled limitations.

The information includes both clinical and non-clinical support content. The clinical content is based on an evaluation of the scientific literature and medical experience; it describes principles of practice generally applicable in most circumstances. If you are posting information on publicly accessible areas of this site, keep in mind that anyone can see it. Do not post any patient or health care provider specific information (such as names, health numbers, or address).

CanFASD does not routinely monitor all material placed on the site, but may in its discretion delete or modify any material offending this agreement that comes to its attention. CanFASD may terminate any users’ access to the site if it considers it prudent to do so in its sole discretion. CanFASD is not responsible for and does not endorse anything users post on the site.

CanFASD acquires no ownership interest in anything you post on this site. You grant to CanFASD a license to use and display for the purposes of the site, and any related uses or promotion, anything you post on publicly accessible areas of this site at no cost to CanFASD.


If you believe that anything posted on this site offends your copyright or other intellectual property rights, is defamatory, or otherwise infringes your rights, you may email CanFASD at the email address at the bottom hereof. We will take actions we believe are appropriate in the circumstances.


Your access to portions of the site requires a login ID and password. It is your responsibility to protect that information, including without limitation, to use effective passwords that are not easily guessed or discoverable, and keep login ID’s and passwords confidential. All promises made by CanFASD with respect to this website are contained in this agreement. No promises implied by law (whether described as warranties, conditions, representations, or otherwise, and whether relating to merchantability, fitness, non-infringement, or otherwise) apply.

You cannot collect any damages from CanFASD for any reason (whether under legal theories of contract, tort, negligence, strict liability, operation of law or otherwise) that are not direct damages or exceed the amount paid by you to CanFASD hereunder for any goods or services that the claim arises from.

CanFASD relies on these limitations when entering this agreement and setting its fees, if any. They are a fundamental and essential part of our arrangement, and apply even if this agreement has failed in its fundamental or essential purpose or been fundamentally breached.


This site is governed by the laws of the Province of Ontario excluding any conflicts of law provisions and excluding the United Nations Convention on Contracts for the International Sale of Goods. Any legal action against CanFASD shall take place in the courts of the Province of Ontario. The parties attorn to the non-exclusive jurisdiction of the courts of the Province of Ontario.


Any third party sites that are linked to this site are not under CanFASD’s control. CanFASD is not responsible for anything on the linked sites, including without limitation any content, links to other sites, any changes to those sites, or any policies those sites may have. CanFASD provides links as a convenience only and such links do not imply any endorsement by CanFASD of those sites.


CanFASD tries to provide accurate information on this site, but errors may occur. CanFASD reserves the right to change the goods and services available through the site and their prices without any notice or liability. CanFASD cannot guarantee goods or services advertised on the site will be available when ordered or thereafter. CanFASD reserves the right at any time to reject, correct, cancel or terminate any order for any reason whatsoever. Privacy Policy

This is Canada Fetal Alcohol Spectrum Disorder Research Network’s (“CanFASD”) privacy policy. This document explains CanFASD's policies for the collection, use and disclosure of personal information. CanFASD abides by applicable privacy legislation.


CanFASD collects information by various methods including information actively provided by its customers, and information derived from user access to our web sites. The types of personal information we collect include names, contact information, IP address, browser type, and when you accessed the site.
The credit card information you enter to make a purchase on this site is not stored. It is encrypted and used only to process the order, then immediately destroyed


This information is used for the purpose of providing our services to our users and customers. If we ever wish to use your personal information for uses outside of those required to provide you with our products and services, we will obtain your explicit permission first.

CanFASD may use aggregate or anonymous information which will not be linked to identified individuals for various other uses for itself and third parties.


CanFASD does not share personal information with any third parties except as disclosed in this policy. CanFASD may provide personal information to its consultants, subcontractors and professional advisers (which shall be bound by privacy obligations) to assist CanFASD’s uses disclosed herein.

If we ever wish to disclose personal information to third parties for that third party’s use, we will obtain your explicit permission first. In the alternative, we will advise that we are going to do so in terms of use relating to the site in question, or we will advise at the point of collection so that you may choose not to participate or provide that information.


Personal information is protected by security measures appropriate to the nature of the information.


Individuals may review their personal information contained in CanFASD files by contacting the CanFASD privacy officer. If an individual believes that any of their personal information is inaccurate, we will make appropriate corrections.


CanFASD may amend this policy from time to time by posting an updated version on our website(s). If such amendments affect how CanFASD uses or discloses personal information already held by CanFASD in a material way, CanFASD will obtain consent.

Notwithstanding the general terms of this policy, the collection, use, and disclosure of personal information may be made outside of the terms herein to the extent provided for in any applicable privacy or other legislation in effect from time to time, or pursuant to court orders.

CanFASD may disclose personal information to another entity purchasing (including for diligence purposes prior to purchase) the assets of CanFASD, provided that entity abides by this or a similar privacy policy.

For this site in particular, the personal information collected includes your name, e-mail address and practice location, including the facility where you will be participating in the program. It will also include the results of any of the activities in which you have participated in relation to the program. In addition, it will include a log of your participation. This log of your participation will contain information about your participation, but not the results of your participation.

Test results, including any quiz and survey results, will be stored in a secure file together with your personal information and will not be released to any party without your personal written consent. Your individual results will be accessible to you only via a password specifically assigned to you. Your test results will be used to create aggregate test reports. These aggregate reports will be available to your facility. Aggregate data and reports may also be used by CanFASD for program administration, program analysis, assessment and marketing. Test results will be aggregated in such a manner that your personal information will not be disclosed. Reports summarizing your participation history including which activities in which you have participated will be accessible to the team at your facility and to any other authority responsible for administering the program for your facility. Activity reports may include your name.


CanFASD may change these terms by posting notice on its site. These terms were last revised on January 29, 2016.
